2020 Fall Soccer

Practices Start Tuesday, September 15th

First game will be Thursday, September 24th

Registration Fees: $45 (non-members) / $30 (members)

Terms: By submitting this registration form, I agree that I am the parent/guardian of said child. I, hereby agree that the Community Wellness Center (including but not limited to its staff, officers, members, or volunteers) shall not be liable for any injury or loss my child may sustain while participating in activities of any kind, whether sponsored by or under the supervision of the Wellness Center. We agree to indemnity and to hold harmless the Wellness Center of any claim whatsoever. I further understand and agree that any insurance coverage provided by the Wellness Center shall be secondary and supplemental to any other insurance I have. I assume all risks and hazards incidental thereto including transportation to and from activities. I therefore, give permission for emergency medical treatment for illness or accident if such a problem arises.

HEALTH AND SAFETY GUIDELINES:

In accordance with our state and local health departments, we’ve put together a list of guidelines and precautionary measures we’re taking to ensure the highest level of safety for our soccer program. We can all do our part to ensure a safe and enjoyable soccer season.

- Limit the amount of personal contact with shared equipment and public surfaces

- Practice social distancing while not actively engaged in play

- Limit physical contact (high fives, handshakes, etc.)

- Equipment will be maintained and disinfected regularly

- Participants are encouraged to bring their own equipment for practice

- Fields will be modified to allow for social distancing

-Concessions will not be offered during the season

- Be proactive, we encourage self-checks daily and staying home when necessary

- Team rosters and number of players on the field for games may be reduced

- Bleachers will not be available for seating this season

- Masks are encouraged, but not required

-Extra hand sanitizer will be provided

- While we do not plan on limiting visitors at this time, if the fields get overcrowded we may limit practices/games to immediate family only. You’re welcome to stay in your vehicle and watch from there as well.
